Job Summary:

Fatima Hospital Sargodha is actively seeking a committed, compassionate, and highly organized Clinical Care Coordinator to join our healthcare team.

As a Clinical Care Coordinator, you will play a pivotal role in managing patient flow, coordinating nursing care, and supporting physicians and healthcare staff across departments including the general ward, emergency room (ER), surgical units, and other inpatient and outpatient care areas.

This role is ideal for individuals who possess strong leadership qualities, effective communication skills, and a deep understanding of hospital systems and clinical protocols. Your primary responsibility will be to ensure that patients receive timely, high-quality, and coordinated care throughout their hospital stay or visit. You will act as a liaison between patients, families, and clinical staff, ensuring that all aspects of a patient’s treatment plan are well-organized and effectively executed. From admission to discharge, your oversight will be key in enhancing patient satisfaction and promoting better clinical outcomes.

Key Responsibilities:

• Deliver safe and effective bedside nursing care
• Monitor patient vitals and report changes promptly
• Coordinate with doctors and support teams for timely interventions
• Maintain accurate patient records and assist in ward administration
• Support infection control and hygiene standards

Eligibility Criteria:

• Education: Diploma in General Nursing or BSc Nursing from a PNC-recognized institution
• Experience: 1+ year hospital experience preferred
• License: Valid PNC Registration
• Skills: Patient care, teamwork, and basic documentation proficiency
